Your First Visit

Thank you for considering the office of Keith W. Mann, DDS for your dental treatment. One of our goals is to provide you with the best dental care available. There are many factors that go into this process so it can happen. It is our obligation to discuss our findings with you and what treatment alternatives exist. It is our desire that you want to learn about your oral health and be an active partner in establishing and maintaining it.

During your initial appointment, we will review your medical and dental history and evaluate several aspects of your oral condition. Part of this initial exam includes x-rays. If you have recent films, you are encouraged to bring them with you. Depending on the type and time they were taken, we may update those as needed. Feel free to bring any sort of ipod device or pillows. Most of our chairs have a massage feature.

Sometimes the first appointment is under emergent conditions. We will still gather pertinent information but we will focus on the source of your emergency. Once that is under control and addressed, we ask that you return so that we can evaluate you and your mouth to achieve a healthier and more stable situation, hopefully avoiding further emergencies. Please call to arrange a time for an emergency appointment.

We are happy to see patients age three years old and above. Some however, may not be happy to see us! We will try. Although we encourage parents to remain in the reception area, we will allow them back potentially to alleviate their or their children’s anxiety.

An integral part of the treatment chosen is affordability. We accept the following forms of payment: cash, check, Visa, Mastercard, and Discover Card. CareCredit is also available to those who qualify. It is an outside credit agency specifically designed for healthcare and offers no-interest payment plans. (www.carecredit.com).  For patients with dental insurance, we are happy to work with your carrier to maximize your benefits and directly bill them for reimbursement for your treatment. However, if we do not receive payment from your insurance carrier within 90 days of treatment, you will be responsible for payment and collection of your benefits directly from your insurance company. We are a preferred provider for several carriers. Please call to inquire regarding the specific plans as these may change with time. You will be supplied with the specifics of our Financial Policy during your initial visit.

Other forms we request your cooperation with at your first visit include a general questionnaire, a Medical and Dental History, HIPAA Policies, a Records Release form for your previous dentist, and Insurance and Financial information. To facilitate the completion of this paperwork, we ask that you arrive 15 - 20 minutes prior to your scheduled appointment time. We will also be happy to mail, fax, or email these to you.
